Telehealth Services
Embark Therapeutic Services, LLC provides therapy through HIPAA-compliant telehealth platforms. Clinical information shared during telehealth sessions is protected to the same extent as in-person care.

Uses and Disclosures Requiring Your Authorization
We will obtain your written authorization before using or disclosing your protected health information for purposes not described in this Notice, including:
